You should ask his doctor, the nurse practitioner or the medical social workers to aid you with this. Why is medicare shutting him down? Do you own a case worker assigned that could aid you sort this out? I know with our Insurance and beside state aide we have a skin worker that makes sure adjectives the paper work is done correctly and go through smoothly.
You can also call the local chapter of the American Cancer Society for proposal and guidance.
Another option may be to contact CancerCare. They are an online non-profit social workers who will counsel you if you product a request. Address, phone, and contact information is on site:

My mother just finished chemo. There be a social worker at the cancer center where my mother have her treatments. They said that if she had a problems near insurance or Medicare that they would help. Is in that somone in that position you could homily to?

alot of times if you drive contained by to the doctors office and it is a surrounded by house treatment the insurance will pay... ask the doctor is you come surrounded by for the medicine if insurance covers?
